📝 Overview
ChiroHD's AI Intake Subjective eliminates double documentation for new patient visits. When a new patient completes their ChiroHD intake paperwork, the system automatically generates a Doctor Briefing and a formatted Full Subjective ready to drop into the SOAP note with one click.
Enabling AI Intake to Subjective
Before using this feature, it must be enabled at the location level.
Navigate to your Live Location, Settings, Office, Office Configuration, Office Options.
Toggle on AI Intake to Subjective
Once enabled, the AI summary will automatically generate for qualifying new patient appointments.
Accessing the AI Intake to Subjective Summary
The AI Intake to Subjective Summary is available in two places.
1. Patient Profile — Documents Tab (View Only)
Navigate to the Patient Profile, Documents Tab, Digital Submissions Tab, AI Summary (appears when all conditions listed in the section below are met).
Use this view to review the patient's information before they arrive.
From this screen, the summary is view only and does not include the Add to Today's Note option.
2. Patient Chart SOAP Note Screen (Add to Note)
To see this view, follow the following steps:
Arrive the new patient's appointment.
Navigate to the Patient Chart SOAP note screen.
Click on the patient in the arrival queue
The AI summary panel opens automatically.
Review the Doctor Briefing and Full Subjective.
Click Add to Today's Note to insert the subjective into the record
Once added, the full subjective is fully editable. You can add to it, adjust the language, or remove anything that does not apply. The final note is always yours — always review for accuracy before submitting.
AI Intake Subjective Settings
The AI Intake Subjective reads a completed new patient intake form and generates a summary panel with four sections:
View Source Intake — A read-only view of the intake form exactly as the patient filled it out.
Watch — Items the AI flagged as key points of interest the provider should be aware of.
Doctor Briefing — an AI-generated summary of key highlights: chief complaints, relevant history, and pain patterns. Designed to get the provider up to speed before walking in the room.
Full Subjective — the patient's information formatted for the subjective section of the SOAP note. Click Add to Today's Note (from the SOAP Notes screen only) and it drops this Full Subjective section directly into the SOAP Note.
Requirements to Use This Feature
If a patient intake summary does not show up, here are four things to look into as to why it might not appear.
