🔍 Finding the Intake Paperwork Link
Navigate to your live location
Go to Utilities
Select Intake Link
📎 For a generic intake link:
Click “Click to copy” under Office Intake Link
❗️Generic link does not expire
👤 For a patient-specific link:
Search for the patient’s name
Click “Click to copy” next to their name
❗️Patient-specific link expires after 30 days
📄 Accessing Submitted Digital Paperwork to Assign to a Patient & Import Demographics
🛠️ Option 1: Through the Digital Paperwork Tab
Click on the Paperwork icon at the top of the calendar screen
You’ll see three sections:
📥 Unassigned Submissions:
Displays forms submitted via the generic link
Click Attach to Patient
Search and select the patient
Click Copy Info to Patient to import demographic data
📎 Assigned Submissions:
Includes forms submitted using a patient-specific link
Already linked to the correct patient
Click Copy Info to Patient to import demographic data
📃 All Submissions:
Shows a complete list of both assigned and unassigned forms
🛠️ Option 2: Through the Patient’s Documents Tab
Open the patient profile
Go to the Documents tab
Click on Digital Submissions
Select the desired form
Click Copy Info to Patient to import demographics (if you have not already done so via the paperwork tab)
