Finding the Intake Paperwork Link

  1. Go to Utilities.


  2. Select Intake Link.

    • For a generic intake link (usable by any patient), click Click to copy under Office Intake Link.

    • For a patient-specific link:

      • Search for the patient's name.

      • Click Click to copy next to their name.


Accessing Submitted Digital Paperwork to Assign to a Patient & Import Demographics

There are two main ways to locate and assign submitted paperwork:

Option 1: Through the Digital Paperwork Tab

  1. Click on the Paperwork icon at the top of your calendar screen.

  2. Here, you’ll see three sections:

    • Unassigned Submissions:

      • Any paperwork submitted via the generic link will appear here.

      • Assign submissions to the appropriate patient and select Copy Info to Patient to import the demographic data.

      • To assign, click Attach to Patient.

      • In the Search for a patient field, type the patient’s name and select them.


    • Assigned Submissions:

      • Any paperwork submitted via the patient-specific link will appear here, already linked to the correct patient.

      • Choose Copy Info to Patient to import demographic information into the patient’s record.


    • All Submissions:

      • This section provides a comprehensive list of both assigned and unassigned submissions.



Option 2: Through the Patient’s Documents Tab

  1. Open the patient’s profile.

  2. Select the Documents tab.

  3. Click on Digital Submissions.

  4. Select the desired form.

  5. Choose Copy Info to Patient to import demographics into the patient’s record.


Tip: If you need to print a submitted form, select PRINT after opening the desired form.

This guide ensures all submitted paperwork is accurately assigned and demographic information is seamlessly integrated into patient records.
