Patients can fill out their paperwork or assessments in the office right on a tablet or computer!
Option #1 -
Navigate to www.chirohd.com
Click on the Login button.
Click on the "Paperwork" option.
Enter your normal login credentials (the ones you use everyday to log into ChiroHD).
Click Login.
Select your office's name from the drop-down.
In the "Please Enter Your Pin" field, type chirohd (in all lowercase).
This has to be done first before being able to search a patient's name. This is a HIPAA regulation.
In the "Search for a patient" field, search the patient's name who needs to fill out the paperwork.
Start New Form.
Select the desired digital form.
Then hand the device to the patient for them to fill out the paperwork.
Once filled out, this will upload directly into the patient's Documents tab (under Digital Submissions).
Option #2 -
Log into you location (like you normally would to operate everyday).
