Patient Subjectives Setup

Configure custom patient subjective questions for appointments and automatically populate SOAP notes.

📝 Overview

The Patient Subjectives feature in ChiroHD enables you to present tailored questions for patients to answer when checking in for their appointment or during treatment. These responses are directly incorporated into the provider’s SOAP note, improving accuracy and efficiency in clinical documentation.


Accessing Patient Subjective Settings

To begin configuration, navigate to your System Dashboard, System Settings, Patient Subjective Settings.


What Are Patient Subjectives?

Patient Subjectives are custom questions you can present to a patient during:

  • Check-in Kiosk

  • Treatment Area (must be using Patient SOAP Mode)

They are configured to display based on:

  • Appointment Type

  • Provider Type

  • Case Type (e.g., Cash, Insurance, PI)


Configuration Options

When setting up a subjective:

  • Where will the patient answer?

    • Check-in kiosk (primary)

    • Treatment area (alternative)

  • What appointment types will this apply to?

    • Example: Chiropractic, Acupuncture, Nutrition, Neurofeedback

  • What case types will this apply to?

    • Example: Insurance, Cash, PI

This allows specific questions for different clinical scenarios (e.g., re-exams, PI cases, nutrition visits).


Creating Subjective Content

Subjective content is built using the SOAP note macro system. This supports:

  • Pain Scale (1–10) – use number range macros

  • Chief Complaints – use multi-select macros to allow symptom selection

You can:

  • Add standard sentence fields

  • Use advanced macros for lists, numbers, and multi-select fields

To add a new subjective:

  • Click Add Patient Subjective Setting, then configure as desired.

Example: You may have separate versions for insurance and cash appointments using the same verbiage but applied to different case types.


Patient Check-in Experience

When a patient checks in via the kiosk:

  1. They enter their phone number

  2. System matches the patient and arrives the appointment

  3. Their assigned subjective questionnaire appears

  4. Patient fills out items such as:

    • “Where is your pain worse today?”

    • “Rate your pain on a scale of 1–10”

    • Multi-select complaints

Once complete, the patient is fully checked in.


Provider Experience

When the provider accesses the SOAP note:

  1. Open SOAP Provider Mode

  2. Click on the patient's name

  3. Subjective answers are displayed.

  4. Click Add to Today’s Note to insert directly into the SOAP note.

This provides a seamless integration of the patient’s self-reported data into the clinical documentation.
