The Insurance History screen in ChiroHD provides a comprehensive record of all insurance-related activity for services billed through a patient’s case. This tool is essential for insurance billers, office managers, and administrative staff to view billing timelines, payment history, and the rebill status of services.


Accessing the Insurance History Screen

To view insurance history for a patient:

  1. Navigate to the Patient Profile.

  2. Go to the Transactions tab.

  3. Click Insurance History.

This will open a detailed breakdown of all insurance services linked to the selected case.


Overview of Key Information

The Insurance History screen displays the following information for each billed service:

  • Service Name and Code

  • Date of Service

  • Third-Party Payer

  • Billing Status

  • Primary and Secondary Payments

  • Write-Offs and Insurance Balances

  • Memo Notes (from EOBs)

  • Date Billed and Date Paid

  • Rebill Status

  • Bill History Log

Each entry is specific to a patient’s active case, and can be filtered or modified as needed.


Filters and Viewing Options

Use the filters at the top of the screen to narrow down the results:

  • Third-Party Payer: Filter by specific insurer.

  • Date Range: Use the start and end date pickers to isolate services within a given time frame (e.g., calendar year 2021).


Bill and Rebill Controls

1. Set All to Rebill

  • Reflags all services in the case for rebilling.

  • Updates their status to purple, indicating they are pending rebill.

Use with caution—this affects the entire case.

2. New Bill

  • Creates a new bill in ChiroHD for all services in the case.

  • Bypasses the Green Sheet validation, which checks for coding, payer, and compliance issues.

This should only be used in specific, advanced billing scenarios where validation must be intentionally bypassed.

Recommendation: Do not use “New Bill” unless explicitly required for a billing exception. Instead, allow services to follow the standard Green Sheet validation process for billing integrity.

3. Set to Rebill (Single Service)

  • Reflags a specific service for rebilling.

  • Click Set to Rebill next to the desired service line.

  • Click Save to confirm.

The service will return to the purple status and be eligible for billing again via the normal workflow.


Bill History and Audit Tracking

For each service, you can view:

  • Full billing timeline including all dates the service was billed

  • Multiple payment history entries, showing:

    • Date billed

    • Date(s) paid

    • Payment source (primary or secondary)

    • Amount paid

    • Memo (EOB notes)

This section ensures transparency and allows staff to reconcile payment activity accurately.


Additional Features

  • Voided Services: Any voided items will display in red, with their full history preserved in this screen.

  • Editable Status: Services can be reflagged for rebill without navigating to other parts of the system.

  • Detailed Status History: Useful for audits and identifying any interruptions in the billing lifecycle.
