How to Process an EOB in the Insurance Dashboard

Overview

In ChiroHD, processing an EOB (Explanation of Benefits) through the Insurance V2 Dashboard allows you to properly apply insurance payments, track outstanding balances, and keep accurate patient ledgers.
This guide walks you through the full EOB posting workflow, required permissions, and best practices.


User Permissions

To access the Insurance V2 Dashboard and process an EOB:

  • Insurance permissions must be enabled for your user account.

  • If you do not see the Insurance tab in your dashboard, you do not have permissions and will need to contact your network admin to enable them.


How to Access the EOB Posting Screen

  1. Click Insurance from the main navigation bar.

  2. Select Process EOB.

Key Fields:

Field

Description

Third-Party Payer

Start typing the payer name (e.g., Blue Cross) and select the matching payer.

Posting Date

Defaults to today’s date but can be changed to match the check date.

Check Number / Memo

Optional fields for internal tracking.

Check Amount

Total amount received on the EOB.

Date Range

Adjust if you're posting services outside the default 30-day window.

Upload EOB PDF

Optional. Allows you to upload a scanned copy of the EOB (this does NOT auto-post).


Important Posting Concepts

  • Remaining Balance:
    Tracks the amount left to post. Should equal $0 when finished posting.

  • Patient Snapshot:
    Clicking a patient's name will show all their open services associated with the selected payer.

  • Case and Policy Overview:
    You can view which cases and insurance policies are active for the patient — helpful for Medicare and secondary claims.

  • Hotkeys to Speed Up Posting:

    • P = Pay in full

    • W = Write off remaining

    • X = Reset line

  • Responsibility Fields:

    • Deductible

    • Co-pay

    • Co-insurance

    • Other Pat (used if insurance denies service and you want to bill the patient)

  • Write-Offs:
    Only write off balances when ALL insurance payments are posted (e.g., after secondary payer).


Posting an EOB: Step-by-Step

  1. Select the payer and enter the EOB details (date, check number, check amount).

  2. Find the patient on the left-hand side.

  3. Click on their name to display open services.

  4. Enter payment amounts and any patient responsibility amounts.

  5. Use hotkeys if helpful to speed up your entry.

  6. Post each patient's payments.

  7. Ensure the remaining balance is $0 at the end.

  8. Click Finalize to complete the posting.

Tip: If the remaining balance is not zero (positive or negative), you must fix it before finalizing. Otherwise, you will need to delete and reprocess the entire EOB.


Handling Medicare and Secondary Payers

  • Medicare Forwarding:
    If Medicare auto-forwards the claim to the secondary payer:

    • Still ghost bill the secondary claim in ChiroHD.

    • Do NOT physically send it again to the clearinghouse.

  • Posting Secondary EOBs:

    • Make sure the secondary EOB is posted only after the Medicare (primary) EOB is posted.

    • Remove or adjust co-insurance balances when secondary pays on behalf of the patient.


Posting Denials or Rebilling Services

If a service is denied:

  • Mark Denied during posting.

  • Add a message to the office if needed.

  • Choose to Leave as Insurance (sends to Green Sheets for review) or Set to Patient Responsibility.

If necessary:

  • Mark for Rebill directly from the EOB posting screen.


After Posting an EOB

  • EOB History:
    View all posted EOBs in Insurance > EOB History.

  • Collections Reporting:
    Insurance payments will appear under Reporting > Simple Reporting > Collections.
