How to Process an EOB in the Insurance Dashboard

Overview

In ChiroHD, processing an EOB (Explanation of Benefits) through the Insurance V2 Dashboard allows you to properly apply insurance payments, track outstanding balances, and keep accurate patient ledgers.

This guide walks you through the full EOB posting workflow, required permissions, and best practices.


User Permissions for Insurance

To access the Insurance V2 Dashboard and process an EOB:

  • Insurance permissions must be enabled for your user account.

  • If you do not see the Insurance tab in your dashboard, you do not have permissions and will need to contact your network admin to enable them.


  • NOTE: Location users cannot submit EOBs (even if they have Insurance Permissions).

How to Access the EOB Posting Screen

  1. Click Insurance from the main navigation bar.

  2. Select Process EOB.

Important Posting Concepts

  • Summary Button.
    Displays every patient listed on the EOB in a scrolling format.

  • Use hotkeys if helpful to speed up your entry.

  • Hotkeys to Speed Up Posting:

    • W = Write off remaining

    • A = Allowed Amount

    • C = Patient Charge

    • E = Deductible

    • R = Rebill

    • P = Paid in full

    • F = Denial

    • X = Reset line

      NOTE: After entering information on any line in the Process EOB screen, this line will lock the information to keep it secure and not changeable. If you need to make a change to a line, click anywhere in the line and click the X hot key to reset the line.

  • Patient Snapshot:
    Clicking a patient's name will show all their open services associated with the selected payer.

  • Case and Policy Overview:
    You can view which cases and insurance policies for the patient — helpful for patients with multiple cases and policies.


Manually Posting an EOB: Step-by-Step

  1. Select the EOB Posting Payment Category.

    Payment Category Options:
    Insurance Check, Virtual Credit Card, ACH/EFT, Remote Insurance Check, Personal Injury Check, Remote Personal Injury Check


    NOTE: These EOB Posting Payment Categories will show as payment subcategories on the Collections Payment report.

  2. Select the Posting Date.
    Defaults to today’s date but can be changed to match the check date.

  3. Upload EOB/Pdf Document (Optional).
    Allows you to upload a scanned copy of the EOB (this does NOT auto-post). This is simply a way to save a copy of the image of the EOB in ChiroHD.

  4. Select the Third-Party Payer.
    Start typing the payer name (e.g., Blue Cross) and select the matching payer.

  5. Enter the Date Range of the date of services to be included to post toward.
    This date range ("From" and "To") would need to include any date of service for any patient that has this payer with any services that are not settled.

    NOTE: If you are posting toward an older DOS, you would need to back this date up to include all DOS that you are posting toward.

  6. Enter the Check Number/Memo (Optional).
    These fields are searchable in the EOB History Screen and can be useful for internal tracking.

  7. Enter the Check Amount.
    The Check Amount will be the same amount as the Payment Amount.

  8. Find the Patient Name on the left-hand side and Click on their name to display open services.

    NOTE: You will see a Quick filter at the top of the list of Patient names that may be helpful to find a patient name more efficiently.

  9. Enter Payment Amounts.


    Specific Fields:

    • Service Date

    • CPT

    • Modifier

    • Type Primary or Secondary Insurance

    • Billed amount

    • Disallowed Amount
      This is generated when there's a fee schedule associated with a patient. The system automatically calculates the variance between the billed amount and the allowed amount specified in the fee schedule.

    • Allowed Amount
      NOTE: We do not control or manage these allowed amounts. They are determined by your Third Party Payers. If there's any uncertainty about these figures, we recommend contacting your Third Party Payers directly for clarification.

    • Payment from Third Party Payer.

  10. Enter amounts of any Write-Offs.

    • Primary W/O

    • Secondary W/O


      TIP: Only write off balances when ALL insurance payments are posted (e.g., after secondary payer)

  11. Enter any Patient Responsibility Amounts as is given by the insurance company, shown on the EOB/ERA.

    Pt Responsibility Fields:

    • Deductible

    • Co-pay

    • Co-insurance

    • Other Pat (used if insurance denies service and you want to bill the patient)

  12. Ensure the remaining balance is $0 at the end.
    Tracks the amount left to post. Should equal $0 when finished posting.

  13. After verifying and correcting any information, click Finalize to post the EOB.
    The posted data will update the patient’s ledger and your collections.

Tip: If the remaining balance is not zero (positive or negative), you must fix it before finalizing. Otherwise, you will need to delete and reprocess the entire EOB.


Common Issues with Medicare and Secondary Payers

  • Medicare Forwarding:
    If Medicare auto-forwards the claim to the secondary payer:

    • Still ghost bill the secondary claim in ChiroHD.

    • Do NOT physically send it again to the clearinghouse.

  • Posting Secondary EOBs:

    • Make sure the secondary EOB is posted only after the Medicare (primary) EOB is posted.

    • Remove or adjust co-insurance balances when secondary pays on behalf of the patient.


Posting Denials or Re-billing Services

If a service is denied:

  • Mark Denied during posting.

  • Add a message to the office if needed.

  • Choose to Leave as Insurance (sends to Green Sheets for review) or Set to Patient Responsibility.

If necessary:

  • Mark for Rebill directly from the EOB posting screen.


Verification After Posting an EOB

  • EOB History:
    View all posted EOBs in Insurance > EOB History.

  • Collections Reporting:
    Insurance payments will appear under Reporting > Simple Reporting > Collections.
