Statement of Account

The statement of account in ChiroHD provides an overall summary of services. The breakdown of services are itemized by the date range selected when querying the statement.

There are currently two Statement of Accounts located within ChiroHD. The Legacy Statement is our original form and provides a general overview of account activity. The BETA Patient Statement of Account gives a more user-friendly overview broken down by insurance and patient activity.

Both reports include:

-Patient demographics
-Pertinent provider information including NPIs and EIN
-Diagnosis codes

Below are the Legacy Statement of Account columns and descriptions:

Date: The date the service was rendered
Description: The CPT code and corresponding service provided
Claimed: The amount billed to insurance by service
Paid: The amount paid by insurance
Charges: The remaining charges after the contractual discounts/allowed amounts and write offs have been applied OR if a cash case, then the charges associated with the service
Write-offs: Any non-insurance related write-offs (ie. prepay or multiple family member discounts…)
Taxes: Any applicable taxes
Payments: Payments made by the patient
Balance: Running amount due

Below are the BETA Statement of Account columns and descriptions:
Column titles will vary if practice does not take insurance

Date: The date the service was rendered
Code: The CPT code
Mod: Any modifiers necessary for billing the corresponding service
Description: The description of the service provided
Amount: Either amount billed to insurance OR amount due for service if cash case
Paid (Insurance Specific): The amount paid by insurance
Write-off: The difference between the original charge and the allowed amount set by insurance
Balance: Running amount due
Owes: Patient responsibility amount due
Paid: Payments made
Balance: Running amount due
