ChiroHD offers three SOAP Note screen modes: Provider Mode, Patient Mode, and Patient Chart Mode. This article focuses on Patient Mode, which streamlines the provider workflow by allowing patients to check in at the adjusting area using their date of birth. This minimizes navigation time and makes it easier for providers to access SOAP note data quickly.
Purpose of Patient Mode
Patient Mode is designed to simplify the provider’s process by enabling patients to check in directly at the adjusting area. It eliminates the need for providers to search through the adjustment queue and improves patient flow.
Accessing Patient Mode
To access Patient Mode:
The user must be assigned the Provider role.
Administrators will not see the SOAP tab unless designated as a provider.
Click the SOAP tab in the navigation bar.
Select SOAP > Patient Mode.
How Patient Mode Works
Once a patient has been marked as arrived, they can enter their date of birth (MM/DD) on a device at the adjusting area.
The system will match them to an arrived appointment for today and open their SOAP Note screen automatically.
If multiple patients share a birth date, the system prioritizes the one with an arrived appointment for the current date.
After login, the system will show a screen saver for HIPAA compliance. Tap or click to remove the screen saver and begin documentation.
SOAP Note Interface (Patient Mode)
Once the patient file is pulled up, the SOAP Note screen in Patient Mode provides all essential clinical tools and information for the provider:
Top Panel
Patient Info: Name, DOB, age
Snapshot: Quick access to full patient details
Call Patient: Signals the patient is ready to be seen
Treatment Date: Defaults to today

SOAP Note Creation
Use SOAP macros to populate content for Subjective, Objective, Assessment, and Plan.
Click Reuse Last Note to populate the previous note for quick editing.
Advanced macros (blue fields) allow real-time editing and updates to variables like pain scale.
Imaging and Documents
X-Rays labeled as such in the profile will appear at the bottom.
Tools include:
Zoom
Side-by-side comparison
Drawing overlays for discussion (not saved)
Quick access to other patient documents for reference or discussion.
Charges and Diagnoses
View default and entered charges in the Charges Box.
Add or update diagnosis codes by clicking into the diagnosis area if blank.
Queue Management
Adjustment Queue
Lists all patients currently arrived and assigned to providers.
Breaks down by provider, with the ability to collapse other providers' queues.
Draft & Incomplete Queues
Drafts and unfinished SOAP notes are accessible and editable from dedicated sections.
SOAP Note Submission Options

Electronic Signature: Automatically stamps the note with the provider’s name, date, and time.
