Provider SOAP Note Screen (Provider Mode) in ChiroHD

ChiroHD provides three SOAP Note screen modes to accommodate various workflow preferences. This article focuses on the Provider Mode, which is designed for efficient, clinical documentation directly within the adjustment queue interface.


Accessing Provider Mode

To access the Provider Mode:

  1. The user must be assigned the Provider role in ChiroHD.

    • Administrators will not see the SOAP Notes tab unless also assigned as a provider.

  2. Click SOAP > Provider Mode from the navigation bar.


Layout and Functional Overview

Patient Overview Panel

At the top of the screen, you’ll see:

  • Patient Name, Date of Birth, and Age

  • Snapshot: Opens the full patient snapshot for additional context.

  • Call Patient: Alerts the patient they are ready to be seen in the adjustment area.

  • Treatment Date: Defaults to today’s date; can be adjusted if needed.


Core Interface Elements

1. Adjustment Queue

  • Shows all patients who have been marked “arrived.”

  • Organized by provider, with options to expand/collapse individual queues.

  • Includes Draft Notes Queue and Incomplete Notes Queue for quick access to unfinished documentation.

2. Cases Panel

  • Displays current case, visit counts, and the ability to switch between cases.

3. Charges

  • Add, edit, or remove charges for services rendered.

  • Adjust:

    • Units

    • Modifiers

    • DX mapping (click + to reassign diagnosis order)

  • View copay, deductible, co-insurance amounts.

4. Care Plan and Pregnancy Tracking

  • Displays the Care Plan Tracker if active.

  • Pregnancy Tracker visible for patients marked pregnant (pink for girl, blue for boy).

  • Updates automatically on a weekly basis.

5. History and Info Panels

  • Visit History: View previous SOAP notes.

  • Case and Info: Jump to case details.

  • Quick Add: Rapidly add preset services.

  • Notes Boxes: Pulls Chief Complaints, Care Notes, and general notes from the patient’s profile.


Using SOAP Macros

1. Creating the Note

  • Click through SOAP Note Macros to input data for Subjective, Objective, Assessment, and Plan.

  • Use Advanced Macros (blue buttons) to enter select, multi-select, narrative, spine, and number range data.

  • Click any macro button to interact and edit in real time.

2. Editing Existing Notes

  • Select Reuse Last Note to repopulate the previous visit's content.

  • Edit dynamic fields (e.g., pain scale) by clicking on the macro entry directly.

3. Signatures

  • At the end of documentation, click Electronic Signature to sign the note with the provider’s name and timestamp.


Document & Imaging Tools

X-rays and Imaging

  • View any x-ray file uploaded to the patient's profile and marked as "x-ray."

  • Tools include:

    • Zoom

    • Side-by-side comparison

    • Drawing tools for discussion only (annotations are not saved)

Other Documents

  • Access any uploaded files directly from the SOAP screen using the Document Panel on the right.


Diagnosis Management

  • View or edit diagnosis codes directly from the SOAP screen.

  • If blank, click the diagnosis area to add ICD codes quickly.


Finishing the SOAP Note

At the bottom of the screen, you’ll find note submission options:

Close

Closes the screen without saving.

Save as Draft

Saves the note to the Draft Notes Queue for later completion.

Submit and Assign

Submits current section and assigns patient to another provider/room.

Submit

Finalizes and submits the note along with the charges; removes patient from the queue.
