By DX Code - All Reports Beta

Learn how to use the Find Patients by DX Code report in All Reports Beta to build a list of patients who have a specific diagnostic code assigned to their case within a selected date range.

πŸ“ Overview

Find Patients by DX Code is a patient-finding tool. It returns a list of patients who have one or more specific ICD diagnostic codes assigned to a case within the date range you select. This report is useful for identifying patients by diagnosis β€” whether for clinical follow-up, reporting purposes, or outreach to patients with a specific condition being treated at your practice.

Like the other Find Patients reports, this report does not contain financial data. Its purpose is to identify which patients have a specific DX code on their case so you can take action on that list.

To access this report, go to Reporting > All Reports Beta, select a location, and click By DX Code under the Find Patients section.

⚠️ This report contains Protected Health Information (PHI).

🩺 How DX Codes Work in ChiroHD

Diagnostic codes (DX codes) are ICD codes assigned to a patient's case to identify the condition being treated. They are added to the patient's case from the patient profile and can have an effective date and an end date. A patient case can have multiple DX codes assigned at once, and codes can be marked as expired when they are no longer active.

When you search for patients by DX code in this report, ChiroHD looks for patients whose case has that code assigned within the date range you specify.

πŸ”½ Report Filters

Filter

What It Does

Required?

Start Date / End Date

Sets the date range to search within β€” defaults to the current month

βœ… Required

DX Code(s)

The diagnostic code(s) to search for β€” you can search by code number or by name

βœ… Required

Patient Status

Filter results to a specific patient status (defaults to Active)

βœ… Required

Export options: PDF and CSV

⚠️ All three inputs are required. Unlike some of the other Find Patients reports where filters are optional, this report will not run without a date range, at least one DX code selected, and a patient status selected.

πŸ” Searching for DX Codes

When selecting DX codes in the filter, you can search by either:

  • Code number β€” e.g., typing "M54" will return codes beginning with M54

  • Code name / description β€” e.g., typing "low back" will return codes whose description includes that term

You can select multiple DX codes β€” the report will return patients who have any of the selected codes assigned to their case within the date range.

πŸ“„ What the Report Shows

The output is a patient list β€” not a financial or clinical summary. For each patient returned, the report shows identifying information so you can follow up with them. It does not show:

  • Which specific DX code triggered the patient's inclusion (if multiple codes were selected)

  • Clinical notes or SOAP note detail

  • Financial information, charges, or balances

  • A count of how many times the code appeared within the date range

πŸ’‘ Common Use Cases

  • Clinical outcome tracking β€” Pull a list of all patients currently being treated for a specific diagnosis to review outcomes or monitor care plan progress

  • Condition-specific outreach β€” Identify patients with a specific diagnosis to send relevant educational content, exercises, or care tips

  • Internal audits β€” Review which patients have a specific ICD code assigned to confirm coding accuracy or compliance

  • Research and reporting β€” Identify how many active patients are being treated for a particular condition

  • Referral follow-up β€” If referrals for a specific condition are tracked by DX code, use this report to pull all patients referred for that diagnosis

❓ Common Questions

  • Why won't the report run even though I've selected a DX code? All three inputs are required β€” a date range, at least one DX code, and a patient status. If any of these are missing, the report will not run. Confirm that all three fields are filled in before generating the report.

  • Can I search by multiple DX codes at once? Yes. You can select multiple DX codes in the filter field. The report will return patients who have any of the selected codes on their case β€” not only patients who have all of them.

  • Can I search by code description instead of the code number? Yes. The DX code search field accepts both the code number (e.g., M54.5) and the code description or name. Type either and ChiroHD will filter the available codes to match.

  • Why is a patient not showing up who I know has this DX code? A few possible reasons:

    • The code may have been added to the patient's case outside the date range you selected β€” try widening the date range

    • The patient's status may not match the Patient Status filter β€” try adjusting the status filter

    • The code may be expired on the patient's case β€” if the DX code has an end date that falls before your search window, it may not be returned

    • Confirm the correct code is selected β€” a similarly named but different code will not return the expected patient

  • Does the date range filter by when the DX code was added or by when the patient was seen? The date range filters based on when the DX code was active on the patient's case β€” specifically, codes whose effective date falls within the range you select. Worth confirming with a test run if you need precise date-based results.

βœ… Key Takeaways

  • Find Patients by DX Code is a patient-finding tool only β€” no financial or clinical detail is included in the output.

  • All three fields are required: date range, DX code(s), and patient status. The report will not run if any are missing.

  • You can search for DX codes by code number or description β€” both search methods are supported.

  • Multiple DX codes can be selected β€” the report returns patients with any of the selected codes on their case.

  • Patient Status defaults to Active β€” adjust if you need to include inactive patients.

  • The date range defaults to the current month β€” adjust as needed for historical searches.

  • This report is PHI-flagged and requires appropriate access permissions to run.

  • Export as CSV for large lists or when you need to sort, filter further, or work with the data in a spreadsheet.

πŸ“Œ Conclusion

Find Patients by DX Code gives your practice a way to identify patients by the diagnosis being treated β€” something that is not easily visible from standard scheduling or financial reports. It is most useful for clinical review, condition-specific outreach, and internal auditing of coding practices. Keep in mind that all three filter fields are required before the report will run, and that the output is a patient list only β€” for clinical or financial detail on those patients, you will need to review their profiles individually.
