Care Plan Templates in ChiroHD allow clinics to define structured pacing and schedule special appointments like re-exams or x-rays. These templates apply only to adjustment appointments and are designed to streamline bulk scheduling of patients over a care period. While not required, care plan templates offer significant benefits for offices aiming to standardize visit schedules and automate appointment planning. This guide covers how to create and configure care plan templates at the system level.

Disclaimer: Although care plans will be phased out in the future, the template used in care plans will remain required for bulk scheduling.


Where to Create Care Plan Templates

➡️ Go to:
System Dashboard → System Settings → Care Plan Templates

You’ll see a list of existing templates or a blank screen if none have been created yet.


Example: Corrective Care Plan Template

An example care plan might look like:

  • 3x/week for 12 weeks

  • 2x/week for 6 weeks

  • 1x/week for 24 weeks

With special appointments:

  • 12th visit → Re-exam

  • 24th visit → X-ray


Creating a New Care Plan Template

  1. Click to create a new template

  2. Name the template (e.g., 24 Visit Care Plan)

  3. Add Phases to define pacing:

    • Each phase represents a block of recurring visits.

    • The order number starts at 0, which signifies the first phase (standard coding structure).

    • Example Phases:

      • Phase 0: 2x/week for 3 weeks

      • Phase 1: 1x/week for 6 weeks

      • Phase 2: 1x every 2 weeks for 12 cycles
        (↳ Use Cycle dropdown to modify frequency)

📌 These phases only apply to adjustment appointments (other types are not supported).


Understanding the Structure

  • The top section (Phases) defines adjustment pacing — how frequently adjustments should occur.

  • The bottom section (Appointments) defines special visit types that should occur at specific points in the care plan only during bulk scheduling.


Adding Special Appointments (Optional)

To add special appointments:

  1. Click “Add Appointment”

  2. Choose which visit number should be a special appointment:

    • Example:

      • Visit #10 → Re-exam

      • Visit #12 → X-ray

  3. Optionally, assign an Appointment Subtype if they’ve been set up at the system level

💡 These appointments only auto-populate when the care plan is applied during bulk scheduling.


Using Subtypes with Special Appointments

If your system has subtypes configured, you can associate them with these special appointments to ensure:

  • Proper labeling

  • Visual cues via calendar colors

  • Accurate time adjustments


Saving Your Template

Once your pacing and optional appointments are configured:

  • Click Submit to save the care plan template

You can now apply this template when bulk scheduling patients for long-term care plans (covered in a separate video).
